  (Customer) hereby provides written notice to Dayton Power and Light (DP&L) that DPL Energy Resources Inc (DPLER) has the authority to receive past, present and future energy consumption records for Customer.  Customer hereby authorizes and instructs DP&L to treat a copy of this Notice as an original. 

In order for DPLER to complete a contract to perform various energy services, this release authorizes DPLER to obtain all energy consumption, rate and billing records, data, projections or estimates from DP&L.

A copy of this Notice and Authorization authorizes DP&L to release DPLER all energy related information pertaining to all of the Customer's energy consuming accounts served by DP&L.  Such accounts are identified above.
